Trekking to Annapurna Base Camp (ABC) is one of the most awe-inspiring journeys in Nepal. Towering Himalayan peaks, lush rhododendron forests, and warm Gurung hospitality greet you along the trail. But as you climb above 2,500 meters, you’ll enter an environment where altitude can begin to affect your body.
Altitude sickness on the Annapurna Base Camp Trek is rare compared to higher elevation treks like Everest Base Camp, but it is still a serious concern that every trekker should understand. In this blog, we’ll walk you through what altitude sickness is, how it can affect you, where it’s most likely to occur on the ABC route, and—most importantly—how to prevent it.
Altitude sickness, also known as Acute Mountain Sickness (AMS), is a condition caused by low oxygen levels at higher altitudes. As you ascend into the thin air of the Himalayas, your body requires time to adjust.
Common symptoms include:
Headache
Nausea or loss of appetite
Dizziness
Difficulty sleeping
Shortness of breath
Fatigue
In more severe cases, AMS can progress into High Altitude Pulmonary Edema (HAPE) or High Altitude Cerebral Edema (HACE)—both potentially life-threatening if not treated promptly.
The Annapurna Base Camp Trek doesn’t reach extreme elevations, but it still involves significant altitude gain in a short time.
Typical altitude milestones:
Pokhara: 850m (2,789 ft)
Nayapul (Starting Point): 1,070m (3,510 ft)
Ghandruk: 1,940m (6,365 ft)
Chhomrong: 2,170m (7,120 ft)
Dovan: 2,600m (8,530 ft)
Deurali: 3,230m (10,597 ft)
Machhapuchhre Base Camp (MBC): 3,700m (12,139 ft)
Annapurna Base Camp (ABC): 4,130m (13,550 ft)
Risk Zone: Symptoms of altitude sickness can begin as low as 2,500 meters. This means altitude-related risks begin from Dovan upward, especially from Deurali to ABC.
Altitude sickness doesn’t discriminate—age, gender, or fitness level doesn’t guarantee immunity. However, certain factors increase your risk:
Rapid ascent without proper acclimatization
Previous history of AMS
Dehydration
Overexertion
Even seasoned trekkers should never underestimate the effects of high altitude.
The good news? AMS is highly preventable with the right preparation and precautions.

Drink 3–4 liters of water per day. Dehydration can increase your risk of AMS.
These substances impair your body’s ability to adjust to altitude.
Carbs require less oxygen to metabolize, giving you more usable energy at altitude.
If you feel symptoms of AMS, do not ascend further. Inform your guide immediately. Rest, hydrate, and descend if symptoms worsen.
Some trekkers carry Acetazolamide (Diamox) to prevent or reduce symptoms. Consult your doctor before taking any medication.
